We invite you to participate in the Florida Alliance for Healthy Communities Opioid Addiction Training and Prevention Survey. The purpose of the survey is to determine gaps in knowledge of opioid core competencies for healthcare providers.

Your participation in this survey is completely voluntary. You may decline altogether, or leave blank any questions you don’t wish to answer.  Your responses will remain confidential and anonymous. Data from this survey will be secure and reported only as a collective combined total.
